COVID-19 Symptom Screening Procedures for Parents
All children who are attending school in person in the morning or afternoon session will need parents to certify that they are healthy to learn, prior to entering the building.
- This will need to be completed every school day before boarding the school bus (bus riders), or entering the school (car riders/walkers).
- We will eventually use an online app for your convenience, but to start, we ask that parents complete a paper form. The form can be opened from home and printed HERE.
- Parents of THIRD AND FOURTH GRADE students who are scheduled to begin participating in the hybrid learning program should use these forms.
- If a child comes to school without a health screening in advance, they will not be allowed into the school.

Plans are underway, but a date is not yet set for inviting 5th grade students to begin the hybrid learning model. Those families will be contacted directly, and the above process will be used.
3rd Grade Hybrid a.m. & p.m. Students: Began Monday, September 21st
4th Grade Hybrid a.m. & p.m. Students: Begin Monday, September 28th
5th Grade Hybrid a.m. & p.m. Students: To be announced--plans are underway--families will be contacted
Remote Only: All remote-only students at 3rd, 4th, & 5th grades continue with a.m. and p.m. sessions. with no changes planned through the first trimester, which ends November 13th.
