Health Requirements Overview

The State of Illinois requires all children to present evidence of having had a physical examination, including all required immunizations.
(State of Illinois Code, Chapter 122, Section 27-8)

• A physical examination (no more than one year old) including immunizations (signed by the parent/guardian and a health care provider) is required for students entering kindergarten and again in 6th grade.
• If a student is transferring into District 7 for the first time, he or she is also required to provide a copy of a State of Illinois physical exam including immunizations (signed by parent/guardian and a health care provider).

Parents must submit the physical examination and proof of immunization forms by the first day of school; students who do not have those health forms submitted will not be able to attend school. If you are unable to schedule an appointment for your child prior to the first day of school, you will need to submit proof of an upcoming appointment in order for your child to attend school.

All Illinois children in kindergarten, 2nd and 6th grade are required to have a dental examination. This important examination will let you know if there are any dental problems that need attention by a dentist. 

• Examinations must be performed by a licensed dentist, and the state dental form must be signed and dated by the dentist.
• Each child is required to present proof of examination by May 15 of the school year.
• School dental examinations must have been completed within the 18 months prior to the May 15 deadline.
•  If you are unable to get this required examination for your child, fill out a separate Dental Examination Waiver Form.

All Illinois children entering kindergarten or upon first entry into an Illinois school beyond kindergarten, are required to complete the below eye examination form.

• Illinois law requires that an eye exam must be completed by an optometrist, or physician licensed to provide eye exams (such as an ophthalmologist). The state vision form must be signed and dated by the optometrist or licensed physician.
• Vision exams should be submitted to the school no later than October 15 of the year the child is first enrolled. The exam must be completed within one year prior to the child beginning school.
• The parent of any child who is unable to obtain an examination should submit a waiver form to the school.

Springman and Attea Middle Schools are members of the Illinois Elementary School Association (IESA). According to their guidelines, in order for a student to try-out, practice, or play a sport, a current completed Certificate of Child Health Examination signed by a properly licensed health provider must be on file the day before the first sport try-outs. Physicals are effective for 395 days (1 year 1 month) from the date issued
